The global response to managing the spread of HIV has recently undergone a significant shift with the advent of ‘treatment as prevention’, a strategy which presumes that scaling-up testing and treatment for people living with HIV will produce a broader preventative benefit. Treatment as prevention includes an array of diagnostic, technological and policy developments that are creating new understandings of how HIV circulates in bodies and spaces. Drawing on the work of Michel Foucault, we contextualize these developments by linking them (...) to systems of governance and discursive subjectivation. The goal of this article is to problematize the growing importance of viral suppression in the management of HIV and the use of related surveillance technologies. For people living with HIV, we demonstrate how treatment-as-prevention’s emphasis on individual and collective viral load is transforming the performative dimensions of embodied risk, affect, subjectivity and sex. Background There has been growing interest in the use of incentives to increase the uptake of health-related behaviours and achieve desired health outcomes at the individual and population level. However, the use of incentives remains controversial for ethical reasons. An area in which incentives have been not only proposed but used is HIV prevention, testing, treatment and care—each one representing an interconnecting step in the "HIV Cascade." Methods The main objective of this qualitative case study was to document the experiences (...) of health care and service providers tasked with administrating incentivized HIV testing, treatment, and care in British Columbia, Canada. A second objective was to explore the ethical and professional tensions that arise from the use of incentives as well as strategies used by providers to mitigate them. We conducted interviews with 25 providers and 6 key informants, which were analyzed using applied thematic analysis. We also collected documents and took field notes. Results Our findings suggest that incentives target populations believed to pose the most risk to public health. As such, incentives are primarily used to close the gaps in the HIV Cascade by getting the "right populations" to test, start treatment, stay on treatment, and, most importantly, achieve viral suppression. Participants considered that incentives work because they "bring people through the door." However, they believed the effectiveness of incentives to be superficial, short-lived and one-dimensional—thus, failing to address underlying structural barriers to care and structural determinants of health. They also raised concerns about the unintended consequences of incentives and the strains they may put on the therapeutic relationship. They had developed strategies to mitigate the ensuing ethical and professional tensions and to make their work feel relational rather than transactional. Conclusions We identify an urgent need to problematize the use of incentives as a part of the "HIV Cascade" agenda and interrogate the ethics of engaging in this practice from the perspective of health care and service providers. More broadly, we question the introduction of market logic into the realm of health care—an area of life previously not subject to monetary exchanges. (shrink)

What are physical quantities, and in particular, what makes them quantitative? This book presents an original answer to this question through the novel position of substantival structuralism, arguing that quantitativeness is an irreducible feature of attributes, and quantitative attributes are best understood as substantival structured spaces.

Recent literature on causation invokes a distinction between deviant and default behavior to account for token causation. Critical examination of two prominent attempts to employ a distinction between deviants and defaults reveals that the distinction is far from clear. I clarify and develop the distinction by appeal to the notion of a modally robust process, and show how the distinction can be employed by causal process theorists to respond to cases of causation by omission. Melvin Woody FREUD'S THEORY OF UNCONSCIOUS mental processes depends upon an extremely narrow conception of consciousness. O'Brien and Jureidini rightly focus attention on the limitations of that conception and argue that it is time to dispense with the resultant conception of the unconscious. Of course, scientists often give narrower, technical meanings to (...) ordinary words like matter and space, but in the midst of all the recently revived psychological and philosophical debates about the nature and existence of consciousness, the persistence of Freud's way of distinguishing the two can only breed confusion. Attempts to shore up or defend the dynamic unconscious of psychoanalytic theory by appeal to discussions of unconscious rules and processes in cognitive psychology attests to the potential for confusion thus spawned. In an early issue of this journal, James Phillips and I charted the differences between the neurophysiologic, cognitive, and dynamic conceptions of the unconscious (1995). Like O'Brien and Jureidini, we concluded that the evidence of a cognitive unconscious does not support the postulation of a dynamic unconscious. O'Brien and Jureidini go further in arguing that the two are incompatible. Elsewhere, I have argued that the Freudian unconscious is a hermeneutic myth, the result of a misunderstanding of the nature of interpretation (2003). Because I am in substantial agreement with them and already on record with arguments convergent with theirs on both these fronts, I will focus here on the source of confusion in the dynamic conception of consciousness and on their illuminating exploration of its limitations.Toward the end of his 1915 essay, "The Unconscious," Freud explains that the difference between conscious and unconscious thought hinges on the role of language: What we could permissibly call the conscious idea of the object can now be split up into the idea of the word (verbal idea) and the idea of the thing (concrete idea)... It strikes us all at once that now we know what is the difference between a conscious and an unconscious idea. The two are not, as we supposed, different records of the same content situate in different parts of the mind, nor yet different functional states of cathexis in the same part; but the conscious idea comprises the concrete idea plus the verbal idea corresponding to it, whilst the unconscious idea is that of the thing alone.... Now too, we are in a position to state precisely what it is that repression denies to the rejected idea in the transference neuroses—namely, translation of the idea into words which are to remain attached to the object. The idea which is not put into words or the mental act which has not received hyper-cathexis then remains in the unconscious in a state of repression. (pp. 201-202) This restriction of consciousness to verbal thought processes is not simply an innocent stipulative definition. It has perniciously misleading implications. For one thing, as Antonio Damasio points out in The Feeling of What Happens, it means that all other animals are unconscious for want of any language. Damasio recalls that when he asked what produces consciousness while he was a medical student, [End Page 155] Curiously, I always got the same answer; language did it. I was told that creatures without language were limited to their uncognizant existence but not we fortunate humans because language made us know... The answer sounded too easy, far too simple for something which I then imagined unconquerably complex, and also quite implausible, given what I saw when I went to the zoo. (Damasio 1999, 107) As Damasio notes, the trouble with explaining consciousness as a function of language is not just that it denies awareness of experience to all other animals, thus perpetuating the Cartesian view of animals as automatons, but that for all its complexities, language is far too simple to encompass the full complex range of conscious experience. In the late nineteenth century and the early decades of the twentieth thematic resemblances to the Roman elegists in Paulus Silentiarius were explained as the result of the poets' reliance on a common Hellenistic source – usually this was identified as the so-called ‘subjective Alexandrian love elegy’ – and this represented a departure from the views of earlier scholars such as Hertzberg and Postgate, who had maintained that Paulus knew and imitated the elegists. In recent years the pendulum has swung (...) back , and the prevailing opinion seems to be that the Roman elegists were known to Paulus. This is the view of, for instance, Giovanni Viansino in his edition of Paulus, of Elmar Schulz-Vanheyden in his important work on Propertius' relationship to Greek epigram, and of Hermann Beckby in his edition of the Greek Anthology. Recently, too, Gordon Williams has put forward a very strong case for earlier epigrammatists like Antipater and Crinagoras imitating the Augustan poets. (shrink)

Literature on testimonial injustice and ways that perpetrators might combat it have flourished since Miranda Fricker’s ground-breaking work on testimonial injustice. Less attention has been given, however, to the role of bystanders. In this paper, I examine the accountability that bystanders may have for their omissions to redress testimonial injustice. I argue that bystander accountability applies in cases where it is opportune for bystanders to intervene, and if they are also sufficiently equipped and able to redress the testimonial injustice. In recent years a novel picture of Mably's thought has begun to emerge within Anglo-American scholarship, suggesting perhaps a constructive alternative to both the �radical� and the �conservative� interpretations. On this reading, Mably should be seen as neither a proto-socialist nor a reactionary thinker, but as a republican -- a classical republican, in fact, whose writing represents a later Gallic contribution to the political tradition founded by Machiavelli and Harrington.3 In fact, nothing is more obvious to any reader than the (...) absolute centrality of the reference to Graeco-Roman antiquity in his writings, from beginning to end -- with precisely that celebration of the �mixed governments� of Sparta and Rome, and intense preoccupation with the politics of �virtue�, that we have come to associate with the civic humanist tradition. The object of this essay is to provide a brief introduction to this �new� Mably. It will do so chiefly through an examination of a single work, albeit a pivotal one in Mably's career. Entretiens de Phocion, a dialogue first published in 1763, introduced in print for the first time most of the major themes of his mature thought; and it remained by far the most popular work to appear in his lifetime. A brief look at it should give us the opportunity to test the cogency of the civic humanist reading of Mably's thought, as well as to suggest something of thespecific character of the French contribution to the �Atlantic republican tradition�. In this paper, we describe Blackboard’s Online Journal program and explain how we use the online journal in a variety of philosophy courses. We outline our pedagogical motivation for using online journals and analyze how online journals help to improve our students’ ability to read, write and think philosophically. We analyze the strengths and weaknesses of online journals in comparison to online discussion boards. Finally, we address several concerns that philosophy teachers may have about using online journals.

Language, either oral or written, is meant both to convey and to preserve meaning. Semiotics is thediscipline which permits the extraction of a meaning from systems of linguistic signs. Written texts arestatic, while the world is about them is in flux. Meaning is thus intimately connected to this marriageof flux and stasis in texts.Here, three views on semiotics are examined:First, Plato’s treatment of signs and flux in the dialogue Kratylos is dissected. The conventional andmimetic aspects of signs are contrasted, and (...) a connection between inquiry and stasis is intimated.Next, the Augustinian theory of signs, as delineated in De Doctrina Christiana, is presented. Thediversity of signs and several principles of hermeneutics are considered. In addition, the existence ofthe some Post-Modern notions of polysemy, trace, and fore-structures of understanding in Augustine’sthought is indicated. Also, the concepts of ineffability and of res summa in De Doctrina Christiana areexamined as starting points for a deconstructive critique of Augustinian semiotics.Third, Derrida's treatment of signifiers in Of Grammatology is analyzed as a turn to a semiotics ofthe flux. Finally, the oral concept of memory reveals a connection between flux and stasis. Thememory plays a role in the system of signs in creating a "generative reconstruction". According toAugustine, sign-making is also "incarnational." Humans retain something of the signs in memory;memory preserves the basis of the sign. This mnemic fundament may be constantly adjusted to thepresent reality, i.e. the flux; the epistemological and semiotic data base in oral cultures is generative orfluid.Memory ameliorates the semiotic tension between the flux of the world and the stasis of information in the system of signs.
